Do medical waivers prevent you from being qualified for RASP?

I'm looking into joining the army and I imagine myself going to RASP; it's one of the factors of why I'm considering the army rather than another branch. I'm working on medical waivers because I've had a shoulder surgery in the past and visited a doctor a couple of times for some mental health stuff. I don't have any current issues, everything was in the past and that's what I'm trying to demonstrate for the waivers.
I've seen online that regiment requires a 111221 or better PULHES. Would I be disqualified with medical waivers? Is there any way to be reevaluated as active personnel?
