Tips/advice for separating from the military (x-post from r/Veterans)

Good afternoon everyone. I am currently active duty Air Force, roughly 18 months from separation with 2.5 years TIS. As time has grown with my time in, I have realized that military/Air Force life does not align with where I want my life to go. With that said, I have been preparing to get out and am currently pursuing a degree in nursing, taking prerequisite nursing classes to eventually get into a program right after I get out.
So, now with a little about my situation, my question to you all is, what is the best kind of advice or tips you can give for someone who wants to get out and wants to make a smooth transition to the outside? I am in the process of making a checklist of everything that needs to get done and want to make sure I do all that I can before my DOS. Any information would be really helpful. Thank you!
